ACR's
Home with 24-Hour Nursing
ACR Medical Services has a home for people who require 24
hour nursing care, including those who are ventilator dependent.
The home is licensed as a Class A facility through the Minnesota
Department of Health, and is open to people who have a variety
of funding sources, including but not limited to CAC, CADI,
MR/RC and TBI. We are prepared to support both adults and
children in this home.
The home is for four people who need 24 hour nursing. At
the present time, we have four residents, both children and adults, three of
whom have trachs, and two of whom are on vents, all have multiple
medical conditions. One is ambulatory, three are not. Our home
is located in Lakeville which is Dakota County. The clients
for this home do not need to be from Dakota County.
There are nursing staff on duty 24 hours a day as well as
support staff. The actual staffing pattern depends on the
needs of the clients. The homes are supervised by an RN as
well as our Director of Nursing, who has extensive experience
with hospital level care and with individuals who are ventilator
dependent. Each individual will have his or her own bedroom,
and activities and programming will be geared towards each
person’s individual needs and interests.
ACR is willing to consider providing Respite Services in
one week intervals to individuals who require 24-Hour nursing
if we have an open bed.
ACR has worked closely with Dakota and Hennepin Counties
to establish standards for the CAC Corporate Foster Care.
As with all our homes, we are committed to providing high
quality services, caring staff, and a beautiful home. We
are pleased to be involved in the provision of high-quality
medical services in a home setting, because we believe this
will greatly enhance the quality of life of the people being
served.
